Case Referral Steps:.
- Download referral form.
- Fill out the referral form. Please ensure no identifying information is included.
- Submit the referral form.
- Email form to Renne Rodriguez rrd23@case.edu
- Family and Children First Council (FCFC) coordinators may also refer a case for consultation through the Family and Children First Regional Liaisons Aimee.matusik@childrenandyouth.ohio.gov or Jennie.horner@childrenandyouth.ohio.gov, or by emailing OFCF@childrenandyouth.ohio.gov.
